We are dedicated to transforming lives and restoring long-term health to individuals, families and communities in Southeastern Kentucky through comprehensive outpatient care, community engagement, and a revolutionary shift in treatment.

Our goal is restoring health, well-being, hope and opportunity to each of our clients.
Education, Prevention & Awareness
Horizon Health is working hand-in-hand with our local and regional partners to educate the community on the disease of substance use dependency. Our #NoShame, #EndTheStigma, and #EmbraceYourStory campaigns aim to increase public awareness around treatment and to encourage others to share their recovery story.
